ORIGIN of SAMPLE:  Tonic/clonic seizure; spit samples collected on cotton rounds and a roll of gauze (ran out of cotton rounds).  Clonazepam given after 2 minutes of seizure activity.  

COMMENTS:   

  • Sample 1
    • hydrated
    • refrigerated between use

Kept sample in container #3, even though the contamination test indicated no contamination.  Remaining 5 containers contained empty canisters for the first time.  NOTE: false indications were on container #5.

SAMPLE:  Wet.  Drained water remaining in canister (very slight).  No sample discoloration.
